Something is really wrong with my hammer

So I’ve noticed over the course of the past few days my hammers polyps have started to shrink up and is starting to look horrible, my parameters are all good. I have had the hammer for 2 weeks

What kind of flow does it have on it?
They really like some good flow. But not to strong.
Did you dip it before you put it in your tank? If so, what what kind of dip?
At the fish store, was it under super bright lights? In the picture it looks like it's at the very top of your tank right underneath some lights. It may not be ready to see such bright light.
What are your nitrates and phosphates at?

it is right under the output and gets decent flow, I used coralRX to dip it. It was under the same lighting, but more blue. My nitrates are low but my phosphates are going to be tested
