Can anyone tell me what this is and how to treat? They weren't there two days ago.

Looks like a bacterial infection, have you done a FW dip to check for flukes? Bacterial infections are commonly secondary to another issue. I would do a FW dip and if you find flukes treat those with Prazi and treat with the trifecta Metroplex, Kanaplex, and Furan 2.

He's in my QT tank, had flukes and ich in display and moved all fish to qt, display is now fishless.
I've been treating the qt with Seachem Paraguard. No sign of flukes in qt and he didn't show any signs of infection or ich until this.

Will Paraguard help this? I'll do a fresh water, ro/di, dip in a minute.

If you find flukes I would use Prazipro and the trifecta are antibiotics to help stop the infection and let it heal.

If you had ich I would recommend using copper power at 2.0ppm for either 30 days or 14 days with a transfer to a sterile tank. This infection could be secondary to ich also.
 
Follow-up
I treated my Naso with API Melafix for two weeks and it's perfectly healed and doing awesome!!!
