Spots on chocolate tang?

I noticed this odd coloration and spots on my chocolate tang today. No changes to the tank or new additions in over 3 months livestock wise. He is still acting his normal self.

Looks like a bacterial issue .
How long have you had this fish?
Focusing on water and diet-
What is your ammonia-ph-nitrate levels?
What test kits are you using?
What foods are you feeding?
This guy will have to be treated in a quarantine setting and best medication would be Seachem metroplex with focus as a binder to the foods assuming still eating
Other option is seachem neoplex.
Add aeration when treating

I’ve had him for about 14 months.
ammonia 0, ph 8.1-8.25, nitrate 15
Hanna test kits and Red Sea
Gets frozen foods. LRS, mysis, brine, and flake and pellet along with nori sheets
Still eating like a pig.

Ok great- go with metro/focus combination.
Maracyn 2 may also work
Nicer looking than mine.
Add formula 2, Hikari veggie diet, and add selcon vitamins and garlic extract to the foods

Should I profolactic lay treat everyone in the tank? Via food?

he may be quite difficult to catch. Plus we are leaving for vacation soon for about 4 days

I would not if inverts and coral present.
What you can use as an Insurance before you leave is ruby Rally Pro which is reef safe and treats bacteria but takes longer and not as effective as aforementioned meds

Can you post new pictures? At first, I thought you were talking about the big discolored area, but now I think that was just a reflection. Are you talking about the little "freckles"? That, combined with the scratching could indicate flukes.

Everyone uses "medicated food" but it cannot work unless you calculate the proper dose (weighing food and binding it with a specific amount of medication and then feeding a specific amount to the fish).

If it is flukes, Prazipro would be the best treatment, that can be done in the DT itself.
