Spots on puffer. Is it a disease??

All fish were quarentined with TTM + prazi and in observation for 3 months then moved to fowlr

The puffer was on a fowlr for 2 years with 4 other fish and never showed and sign of disease. I moved it a couple of months ago to a tank that was with out any fish for at least 6 months and now I am seing this dots all over him.

He eats and behaives normal..

Any ideas of what could it be??

Looks like Ich (or possibly even velvet) to me. The clouded eyes are also an ich symptom

Looks like ich to me. I have treated porcupine puffers with hyposalinity and coppersafe.

The tank is full of aptasia not sure if aptasia can irritante the fish and cause these dots???
The puffer was fine in fowlr tank and the tank was fishless more than 6 months...
I can not see how it could be ich.

In addition the tank is outdoors and at night Temp can vary a lot.

Any thoughts?
