Starry Puffer "coughing"?

So I didn't see any signs of flukes in the FW dip. Can anyone see anything in the pic that could help diagnose the problem?
 
So I woke up this morning and noticed the trigger doesn't look good now when yesterday he was perfectly fine. I have since put him in Quarantine that has Cupramine just in case.

I'm thinking maybe there's a parasite in the main tank, or is there by chance that when the puffer died it released a toxin in the system?
unlikely, you can put carbon if you want, but it would only be released if the clown was cut up. what are the tank params? maybe something is off?
 
I noticed his fins are frayed, but idk if that's because he was sick.
 
Im sad to say that the trigger did not make it.

I thought we were making progress, but unfortunately he passed overnight.

I'm wondering if the problem was something else, and now I'm afraid to restock the tank.

Should I do a complete teardown, bleach the live rock, and replace the substrate?

Thanks, yeah I was hoping he'd pull through. Now I'm trying to figure out what to do with the tank. The tank is empty now, and I don't wanna add new fish if the tank is contaminated

The safest, if you don't want to do a complete restart, would be up the temp to 81 and let it sit for 45 days.
