I have a 55-gallon FOWLR tank that has been running for years now. I have long-term residents of a Foxface, longnose hawkfish, sleeper banded goby, a flameback and a clownfish. In the way on inverts I have 2 conchs, 10 blue leg hermits, and 3-4 nassarius snails. Do you think the tank is at max compacity or can I add another fish (I would like a small puffer like a blue spot.)

You could probably add a few small fish that occupy different parts of the tank. Royal gramma, midas blenny, Fairy wrasse, etc...

But I think a blue spot would be fine
 
Would need to see a full tank shot to make this decision. Amount of fish is dictated a lot by hiding space available vs how many, in what.

Currently no where for fish to sleep or hide. I wouldn't add any more fish. They need caves and hidden areas for sleeping and a place to call their own.
 
They mostly go into the shell and that center piece has some big holes in it. Think I should rearrange the tank though?

I would add more rock rather than rearrange. Create some places for fish to hide. Each fish needs their own spot.
