Sudden Torch Deaths

I have a 45 gallon jbj tank with a good amount of euphylia including three torches. About a week ago i noticed that one of the heads on a torch was receding and looked pretty bad; the next day it looked like it had a white covering on it and it was dead. All other heads on the torch look normal. I tested temp, phosphate, Alk, magnesium, and calcium and all are within normal levels. The only difference i have noticed is that there seems to be some sort of film on the surface on the water recently. Also i should mention that i dosed flucanozole about two months ago and i also have recently begun removing sand during water changes slowly starting a month ago, however i see no adverse affects on any other coral. I have not done par readings but here is the schedule i am running.

No other stinging coral nearby?

I’d rule out a systemic prob if it’s just the one head.
 
Ya know, I've had torches do that. Had a massive one a few years ago. Had it for 3 years and it was amazing. Then one day, the heads started to brown jelly and I lost the whole thing within a matter of 2 weeks or so. All other euphelia we're good.
 
Haven't heard of the fluconazole affecting euphillia at all.
 
Thanks for all the replies, i have had all of them for at least a year, no other corals near it except other torches.
 
I haven’t lost any other heads yet, but they don’t look good. It almost seems like the tissue around the head turns white and the mouth of the head is gaped open and polyps are deflated. Is there anything i should do? Maybe change carbon and do a water change???

also should i dip them in coral revive?
 
Lost part of a different torch and part of a frogspawn. All corals including sps seem unaffected besides the euphylia
