Swimming Worm (2)

I am reposting this with a link to YouTube video of worm that I caught swimming in tank. Anyone able to identify it?

My iPhone had a hard time focusing on the guy but I think you may be able to see enough to make an ID on it.

He is 'pointed' at both ends and somewhat more 'wormish' in the middle.
