Tang and domino damsels Dying HELP

Hello

My tank which I’ve had for 2 years now, I added two Tangs - yellow and blue couple of months ago

The blue tang and my domino damsels(which I’ve had since the start of my tank) all started turning white and scaly.

The blue tang died yesterday and one of the domino damsels died yesterday I’m not sure why - I do water changes every week and clean the tank every week and have a protein filter.

Could someone please advise, will I lose all fish?

It seems if you can kill a domino damsel, your water quality must be really off imo. List all parameters

Yuck, that looks really awful. Are those freshly dead or slightly decomposed?? Looks like the bottom of a Petco fish tank. Any new additions recently? Anything wet counts. (Corals, inverts, macro algae...)
