I couldn't identify that for you. But I have several tangs that have had similar blotches and plenty of other unidentified parasites/injuries/infections that just went away quickly. After a while, I stopped bothering to look them up unless the blemish persisted past a week or got worse. If you don't get an identification from someone else, my advice would be to monitor it and see if it is getting worse. if it does, remove the fish and treat it for a broad spectrum of ailments. I did that once for a mystery infection and it went away in a few days of quarantine with medication.
 
Question is, how long have you had it? If you have had it for a while like more than 3-6 month, mostly likely not ich... Kole tangs are curious and clumsy IMO, and they will have body blemish either for swimming clumsy and hitting a rock, or curious for fighting with other fishes... May have fought with your Six Line Wrasse or Royal Gramma...
 
Thanks. He's only been around a few weeks. I'll continue to monitor and if he looks like he is getting worse, then I'll do a fresh water dip and then quarantine.
