I've had this tang for a few years and then one day he had a lump/scratch on the side of his head. Did he run into a rock? get bit by my hawk fish? infection or is it some disease? He did get a few ick spots about a week later?

Any ideas?

Hmmm I’m not so sure. Has this gradually worsened to this? Part of it looks like lymph but this is not a typical location/presentation of lympho. I’m leaning toward a growth perhaps even infected.

yes, it has gotten worse over the past week. His face is has gotten pale. He was eating fine but today no appetite and less active.

I would not use any antibiotics dosed into the water of a reef tank. The effects on corals and inverts can be rather harsh.

Sulfaplex aka Triple Sulfa works pretty darned good on these types of infections on the head of fish.

^^ I agree with Lympho diagnosis, as I have seen it present this way before. However, dosing a sulfa drug in this situation is a wise precaution.

unfortunately I won't be able to catch him in my tank. maybe a cleaner shrimp can help?
