Tank Possibly Contaminated?

3 weeks ago I purchased a davinci clown fish, and since day 1 he has not been quite right. He took a entire week to eat, after that would only eat one or two pieces of food, and it would take him a good half hour to find it. The only thing I can think of is that from the shop he just simply had problems. The only thing I add to my tank is 6ml of Microbacter Clean after my weekly water change. My main worry was that somehow I infected my tank with bayer. I use to dip my coral. However. After dipping I let the corals sit in a cup for a good 5 minutes with tank water. After I agitate them pretty good. Sadly today he became so weak he kept getting sucked into the overflow wier. It has been 3 weeks, and he finally passed. The tank levels are fantastic. no ammonia,nitrite, and 3 ppm nitrate. All corals I have are happy. The salinity is 1.024-1.025. Should I perhaps run a sack of carbon for a few days to make sure everything is ok. Although I do not know what could be wrong honestly. This just has me stumped. He is the only clown fish I have ever had that has acted like this.
 
The clownfish could've been in bad shape when purchased, possible internal parasite. I don't think your tank is contaminated and would not give up.
Oh no plans on giving up my friend. Just worried that i may have contaminated the tank. Thank you so much for your reply n.n. i am quite worried.
