This doesn’t look good

I came home after an outing for a couple hours and found my anemone like this. It looks like it moved a little and his tentacles are semi puffed up. Any ideas of what is happening here? Parameters are good. I did do a water change today and am slowly changing my salt from instant ocean to Marin tropics.

Without seeing how it looks from the front it’s a guessing game but to me it looks like it was on the move and fell off the rocks. I’d say either leave it and monitor it as it may just continue to move and settle somewhere or you can gently pick it up and place its base (which should be sticky) onto a rock (hold it in place for a couple of seconds until it sticks) and then let it continue on its way.

Biggest thing is you don’t want it near a power head and by the looks of it looks like it hasn’t been hit by anything.
 
looks fine, the mouth is more protruded then usual but maybe that’s because it was upside down. Just monitor it for now
