I’ve have this fish in QT for about 2 months. It eats clams, mussels, oysters very well out of the half shell. Won’t accept food from the water column though. At first I thought it was just a bit of lymph. But it just keeps getting worse and not better.
So far I have tried:
FW dip - no effect
Metro - no effect
Kanaplex - no effect
Cuprimine initially to proactively treat for Ich.

I’m kind of out of ideas. Pics aren’t the best but it was the best I was able to get. Basically white patches all over the side of the fish. Doesn’t appear to have any ich. Fish is very alert, active, and appears to be acting totally normal. No scratching, nothing.

Any ideas are appreciated

Unusually severe lymphocystis? If it's been in qt with this that long and still otherwise healthy and eating, I'm not sure what else it's likely to be. I'm not certain, though. I see why you're probably hesitant to transfer it to the dt, although if that is lympho, the presumably superior water conditions in the dt may be just the thing it needs.

Looks lympho. @HotRocks

Never seen lympho that bad but it does resemble it and the history fits. I don't see any lesions on the fins, are there any?

Could it be a fungal infection of some kind?

I initially thought lymph but I have never had it continue to get worse instead of slowing getting better. Fins are totally clear.

Definitely not ich. Pics aren’t the best but I’m unfortunately way to familiar with ich and this isn’t it.

I considered something fungal but never really had a problem with it in salt water.

Could be bacterial, but OP stated he used Kanaplex and metro. Could it be resistant?

Kyle, do you think worms did this?
I have a blue face angel and emperor in QT right now that looked similar. 2 days after dosing general cure they are back to normal. This flared up as soon as they finished copper. Flukes can actually cause quite a bit of external damage by biting. Pics below:

@jsvand5 I would try two rounds of general cure or Prazipro 5-7 days apart. Dosed into the water.

This video is not even 24hours after first dose of GC. You can see how much they have improved.

I did a fw dip with no change at all and nothing falling off the fish. so I’m not thinking flukes.

I did cuprimine at .5 for 4 weeks. Test with seachems kit.

I’ll give general cure a try. It does look similar to what is on your angels.

We are nuts, @hotrocks’ turn is next!

If no “sesame seed” looking things came off it’s probably safe to assume that it wasn’t flukes. Let us know what general cure does.

Well it looks like general cure was the ticket. It’s about 90% clear now. Thanks for the help. I was ready to throw in the towel on this one.

Glad it worked. Flukes are probably more common than you'd realize. I never would have thought they could inflict the damage they did on the bluface that I uploaded a pic of above!
