Tips on keeping pipe organ corals?

I used to have a pipe organ when i first got my 120 gallon tank running about 7 years ago, and it thrived for a while, grew out and did quite well for about a year or so, before it suddenly seemed to crash. All the polyps retracted and then it just died. My tank was going through several transitions around that time, so it didn't surprise me that some corals just died. (My softies did well, my monti grew out and my birdsnest continued to thrive).

After finishing a battle against cyano and hair algae and having my parameters back in place for a couple months, i decided to try again on the pipe organ. Bought a frag from WWC about 3 weeks ago, and at first several polyps were out and doing their thing, but by last week only 1 polyp from the frag was out, and by today none are out at all.

My parameters are nothiing special- Alk are at 8.5, etc. Any ideas on what i should be looking for to improve my chances on a pipe organ? I would really like to have a good colony going that thrives. I dose All for One and Kalk. I placed it in the lower third of my tank, there was good decent flow, but after only 1 polyp was out, i tried placing it in an area with less flow, but no luck.
 
Moderate light and medium to moderate water flow
Salinity 1.025
Avoid elevated phosphate levels
 
my phosphates are in control in my salinity is right in line. Or do you think i need to bottom out my phosphate?
