Toadstool help

This is a japense neon toadstool.
It’s been in my tank since the beginning (like 4 months) and has was originally doing well, but has recently shrunk and is looking very sad
Initially I thought it was shedding, but it’s been more than a month or so now of it looking like this

Params are stable and all other corals are fine!
Only change I did was remove one out of the three ai primes I’ve had about 2 months back
Please help! Thanks in advance

i have one that did that, off and on. moved it up to higher light and a little more flow and its doing well.

Just ripped it off the rock? He’s attached on there

mine was on the sand bed, now glued to the rock but still on plug so i can remove it if needed.
