Tomini tang - should i be concerned?

I know Tomini tangs have white marks just below their eyes but ours has a couple above the eyes too, we've had it a few weeks and there were only the normal lower white marks when we got her ( we think as we only have one not great photo of her at that time)

The extra markings seem symmetrical too

No other fish show any of these signs and we do run a uv steriliser on 24/7 at a flow rate for fish disease.

Any thoughts

It looks like a new tank...less than 3 months old.
New fish in new tanks just got more stressed. Stress can bring on a little case of ich. Assuming you did not quarantine them, which is fine in my book. You need to find 3 or 4 foods the tomini and foxface love to eat and are quality nutritious foods and feed them small meals 3 or 4 times daily.

I would suggest 2 quality pellets like Hikari Seaweed Extreme and NLS. A good flake food like Ocean Nutrition Formula 1 or Spirulina Flakes. And PE or Hikari frozen mysis. Enriching foods for 1min with Selcon before feeding also helps.

Finally have you tried Nori on a clip or rubber banded to a rock? Enrich the Nori in Selcon (or Vitachem or Aminomega) to add vitamins. Tangs like to constantly pick algae off rocks and you haven't yet developed turf algae for them to enjoy.
 
Tank is just over 5 months old, there really is no issue with any appetite in the tank, tang and foxface eat nori off a clip and frozen and the tang is constantly pecking off the rocks all day

The two lower spots are the fish's nares (nostrils). The upper dot is too large to be ich, more likely a scrape or a bump. I would just watch it for now.
