Do you have a breeding clownfish? If so, I’d say that looks like a possible bite mark healing up.

if mine gets too close to where she’s laid eggs, she lets the tang know! The scars on the tang look just like that and heal in about 3 days.

Include a short video in white light. Then look at the sticky post from @Jay Hemdal and include the information he asks for in there.

2 fairy wrasses, originally dominated by flasher wrasse, now push him around

1 flasher wrasse, has irrational fear of tang, went into hiding for a couple of weeks, doesn't look good but has now come out and slowly recovering.

1 spot fox face, slightly smaller than tang.

Cherub angel, small and only chases wrasse once in a while

Mandarin grown from baby

Tomini tang, went in and out of qt with foxface, angel and eightline flasher that died in qt.

Qt for all fish except mandarin was 10 gallon with lots of pvc and a bit of rock, no aggression from anyone.
Ramped copper to 2.5-2.6 and left for full 14 days before transfer straight into DT.
Didnt realize tang had small white splotch on tail until he went into DT already.
White splotch slowly disappeared and body developed what you see now over a few weeks. He did do a bit of tail and fin twitch but stopped after the splotch was gone. He now does a bit of a curling twitch to his right side once in a while, almost like he's trying to look at the spot on his body.
All other fish look and act fine, until the flasher got spooked and hid for a long time in his rock hole. When he came out there's a bit of white stuff on his body but I'm thinking it's mostly mucus from being holed up for so long. He's recovering and slime is disappearing.

All fish eat well. I feed LRS and bloodworms, soaked generously with aquaforest Fish V, once or twice a day depending on my schedule. Also white worms every day and tisbe pods every couple of days.
Tank never really had algae so I feed Nori everyday/every other day.

All fish breathing normal except foxface in qt, but he was constantly freaked out by my presence, now looks normal.

No fish losses aside from eightline in QT. He died on last few days of copper, and did not look like he had anything wrong on his body. Kind of mysterious as he was behaving normally the day before death, ate ok as well.

No clowns, will be adding when I break down my other tank though.

I see the large lesions on both sides of the fish. Their large size, and being on both sides, tends to rule out an injury. They also are not any of the protozoan diseases, and it doesn't look like flukes, since the lesions are concentrated in one area. That said, I can't tell you what this is. It is probably a generalized bacterial infection, but I can't rule out a fungal issue.

So the lesions are slowly growing larger over a few week's time?
One idea would be to move it to a QT and dose with antibiotics. However, with the foxface in there now, it might put the tomini at risk to move it over there.

What do you mean put the tomini at risk? Like when I put him back in DT there would be aggression?

Thinking about it I think there was a slight discoloration that was the lesion on the right side when he first went in DT. It's gotten darker and a bit bigger.
Left side is new though.

Also are there over the counter meds I can use for human? I'm in Canada and the only meds I have left are a bit of furan 2 and a bit of metro.

The tomini would be at risk as far as catching anything that the foxface might have being only partway through the quarantine process.

I would try a broad spectrum, gram negative antibiotic. Furan2 fits the bill, but you'd need enough for a full treatment. Human antibiotics are tricky - some have excipients and binders that make them not usable in aquariums.
