Our torch was doing really well, it was one of our most beautiful corals. We moved and moved them all to a new tank. Tests all look good and our hammers and other torch are doing great. This one closed up and appears to be dying. We have no idea whats wrong. Any advice? Hopefully it isn't to late.

Looks like the start of polyp bailout. You say your levels are fine but what are the numbers? Has flow changed? Has lighting changed?
 
We are dipping it. Looks like flatworm. I can't upload a video
 
Even if it does die leave it in your tank . I’ve had many resurrected from the dead after months dead
 
Thank you for the dip advice. Hopefully you saved us from this spreading to others. We don't see it on anything else.

Get a Yellow Corris Wrasse, they will keep your tank mostly pest free. They are the perfect size for any tank.
 
We have a six line wrasse that we got for bristle worms. He didnt seem to notice the flatworms
 
It looks like it was sucked tight into its skeleton probably mad from the move. I would watch it but not bug it too much. I wouldn't dip it anymore as dips can actually be hard on corals. If you see recession and it turns Into brown jelly then I would be concerned but corals can almost disappear when they are in shock
Edit: not sure about the flat worms but if they are bugfing the corals flat worm exit is a good option.
