Torch got decapitated

I thought something ate my torch because the stem and plug are still there..Then I looked right below and there it sits. Couple of questions.
1) What could of caused this?
2) What do I do with it now?
3) 3rd unrelated..What are these called and are they bad?

I did have to move my newish BTA rock towards the front of the tank because it was stinging my red finger sponge. This did put it closer to the torch but not that close. Could this of stressed it? What do I do now?
Can you identify pic 3?
 
If the nem stung it its possible that caused it but from the way you put it it seems they werent close enough to touch.

You can try placing the hammer polyp somewhere it wont get blown around, there is a chance it could reattach to rock work and survive, but in most cases they will die out.

The 3rd pic looks like a ball anemone to me. They are a pest and i would try to remove it.
 
Polyp bailout as far as I know is something that happens over time not an immediate reaction. Most likely parameter, flow or lighting related.
