Torch problem

Hi,
I have a pretty simple reef, mostly softies , leathers, Duncan’s, Montis and two torches.
I’ve been keeping them for about 1 year now and everything is fine. My parameters are stable. I’ve had a few aiptasia so I added a small file fish who promptly got rid of them. The fish are well fed.
I have kole tang, one spot foxface,Valentini puffer, boxfish, cowfish, melanurus wrasse and various crabs etc.
I go to work this morning and the torch is fine. I come home 12 hours later and 3/4 of the tentacles are completely gone, right down to the skeleton. I’m sure there’s no saving it now, but what are your thoughts on what happened. I also have 2 urchins.
None of the creatures have ever shown any interest in the torch or other corals.

Puffer or filefish.
I have blue spot puffer and 3 torches. He's never bothered them yet.
 
Our valentini puffer and filefish both decided to start nipping at corals, especially LPS. Both had to be returned. That world be my guess.
