Torch suddenly lost head

I noticed a group of torch tentacles wedged against a rock. It seems 1 of the 11 heads of the torch had a polyp bailout.
All parameters are good and this torch started out as 1 head.
Anyone else experience this?
Torch is not near other corals, so I don’t think any aggression took place.

Not able to, the head that lost tentacles is in the center.

In my experience other polyps did the same until I removed the dead ones.
