Torn fin ?

So when I first got my yellow tail damsel I noticed he was a bit discolored. When I look at him again his fin on his side and tail is torn up . Is this a fin rot sickness? I really hope it’s not .. my other fishes are doing fine but.. he is bit discolored and has a torn fin. What should I do ??

Should I remove him from my display tank ?

That’ looks like fin rot. Teramyacin or even melafix should fix this. Assure tank conditions are good which in itself is a preventative

Can I just put the melafix straight into the tank or do I have to separate it?

In tank good. Just follow dosage on bottle
