Vermited Snail??

If so, how do I remove?
If you don't have too many you can break/scrape them off. If you have any worm eating wrasses they might come by and take care of business after you break the big stalk. I have bumble bee snails in my tanks, whether or not they are helping I'm not really sure.
 
Those look nice and exposed so you should be able to break them off and get the whole snail.

When they're not easily accessible, I've "heard" that if you don't get the body of the snail, it'll just grow back so some people super glue the tube opening to starve them.
 
I broke down my tank and started over because of those jerks... I fought them for two years and it just got worse and worse... i had thousands of them by the end...

Do not let them Sit there and spawn. Cover them with Super Glue and seal them in... scraping them off does not work as that is what i tried in the beginning.

Once they get in the holes in the rocks and overflow where you cant see or reach you are screwed.
 
Try out a couple bumblebee snails. I had a BAD outbreak of vermedits about 8 months ago. Bought two bumblebee snails for my 75g, and not I can only find one or two verms. I never saw a bumblebee snail eat one, but I added em, and it worked.
