Wall Hammer coral die/rotting

Hey guys I'm quite new to reef keeping and experiencing a problem. I've had this hammer for over 3 weeks now and it start this rotting/dieing. The polyps were always out but yesterday I realised they were not out and today its dieing! Can anyone give me advice to maybe save it.

You said cycled. What is ammonia and nitrate readings?
Type of water- RO or tap water from faucet?
 
Yes it's cycled.
Ammonia is zero and nitrate is 20ppm though. Did a water change to lower a bit

did you dip the coral when added? if not do so now. if you expand the last pic posted it looks like there is a polyclad worm eating it on left half. but it may be beyond saving
Yes I did dip it when I bought it.
Really how can I get rid of that worm?

Sorry I'm new to reef keeping... Is the polyclad worm that orange?
 
Nitrate may have an effect. I highly rexcommed to dip that coral as that may very well be a flatworm.
CoralRX, seachem dip, bayer. In an emergency- peroxide but peroxide may or may not work.

Okay thank you for the help. Could fragging it help or rather just leave it alone?

Leave it for now. Elimination what looks like a flatwork and lowering nitrates may display some good recovery
