I use Aquarimate, it's easy, has all of the above and you can export to excel. Are you looking exclusively for an excel spreadsheet?

I use a spreadsheet I found on a forum months ago. I'm not sure if its what you're looking for, but it tracks all my tests for about a month and puts them on graphs for me. It's only a month though, but it saves the data and you could always just copy and paste the data into a new workbook to make graphs for a longer time period.
I couldn't upload the file, so here's the google drive link:

Let me know if it doesn't work. You have to download the file to your computer. It won't let you preview it on drive.

Would not work

If you're using excel it's easy,
Calculate the average of numbers in a contiguous row or column
  1. Click a cell below, or to the right, of the numbers for which you want to find the average.
  2. On the Home tab, in the Editing group, click the arrow next to AutoSum , click Average, and then press Enter.

Tracking water parameters can be tricky, especially when you’ve got gaps in the data. I once faced a similar challenge with my aquarium, where I needed to keep tabs on water quality over time but missed some testing days too. As for a maintenance schedule checklist, I added a simple column in my spreadsheet with reminders for each task, like when to clean filters or check pH levels.

I ended up making an Excel spreadsheet to handle averages and fill in those blanks. It was surprisingly easy to set up, especially after I checked out MyExcelOnline for some Excel formulas and tips. They had a bunch of stuff that helped me figure out how to manage my data.
