Weird looking splatter hammer polyps

Hey all! Wanted to see if you all had any recommendation or reason why my splatter hammer is looking weird. It’s been about a month it has looked like this and not sure what the issue is. Water perameters are as follows:
Salinity 1.025-1.026
Nitrates 10 ppm
Alk: 9.5
Phosphates: .05 (having issues keeping consistent)
Calcium: 420
Mg: 1380

i don’t see anything out of the ordinary. All other corals are acting normal. Having issues with the splatter hammer and a torch that stays retracted (you can see the torch in the video). I have dipped them in coral rx as well as iodine dip and no success. What could be going on?

Pics are dark and looks like it’s extending for more light
They are best in moderate light and water flow at mid tank or a little lower and 2-3 feedings weekly
Calcium at least 380-400, not below
Polyps should not bounce around vigorously or be bent over skeleton
