Nitrate is 10 and phosphate is 0.5

Flow to high possibly! I have the exact one and it doesn’t like flow like my other hammers next to it.. one day I noticed a couple gold balls floating around my tank and was like huh? Haha.. I had to move it and now it’s happy..

I have a peach hammer that started looking worse and worse…. I finally pulled it out and scraped about 2-4 vermited snails off the skeleton and dipped it in Revive. It has helped tremendously.
The pictures below show what it looked like when I pulled it and dipped it… the last pic is right now-( the lights just kicked on 10 min ago so it still looks rough but it has been opening up much more throughout the day). I pulled it 1 month ago. So you might really inspect it and or dip it.
