What’s wrong with my Splatter Hammer?

Need some more experienced members to help diagnose. It’s been slowly retracing for about 2 weeks now- looks like it now has two heads. Other hammers in tank are fine. Does this happen when they grow a new head?

DKH- 8.3
CA- 440
Nitrates 20
Phosphates .09

I would try different placement to start with, and watch the piece closely for brown jelly disease. The coral should not remain retracted when forming a new colony and or new growth.
 
Thanks Tahoe- I have moved it twice already with higher flow and lighting- low flow and lighting. Same results..
