What are these egg sac looking things on this fish???

Dont combine IMHO. wait/watch. @Jay Hemdal will weigh in. Dont jeopardize the rest of your fish for 1. IMHO - this looks like severe / significant lymphocystis. How is the fish doing otherwise?

The fish seems fine. I tried to give a brief freshwater dip and attempted to see if these sacs come off easily. That stressed the fish out, but it was fine shortly after this. No heavy breathing. The fish is eating too

Puffers are weird IMHO - dont jeopardize the rest for one. If you can get a second QT tank - but - how is the fish doing - eating, etc - now.

IMHO - stop worrying - keep feeding - he/she will either make it or not - I know that sounds callous - but - just dont jeopardize the rest of your tank for one fish. Keep feeding well - except for the sacs - he looks awesome. I've always wanted a puffer

PS the fish is in a store system with a giant UV (but with very fast flow going through it), 0 nitrate, unknown pH (I can check next time I am there), 0 ammonia/nitrite, a lot of phosphate, salinity of 1.023 I think, and copper power between 2-2.5ppm. The system is well established. The system receives no water changes, has a a tiny skimmer with biopellets, sponges, an algae scrubber, and thats it.

In another comment I mentioned the fish is eating and breathing normally. Sadly, setting up another qt is extremely unlikely to happen (I don't have control over that).

Looks like Lymph. Best coarse of action if to use a lot of Vitamin C. It will boost the fishes immune system and fight off the infection. It is a virus so think of it as being similar to the fish having warts. Best to soak freeze dried krill with vitamin c to ensure the fish is getting a good dose. Try doing this for a few week and you should notice they begin to shrink.

Lymphocystis always involves the edges of fins, even if it is on the body itself in spots. I don't see Lymphocystis here at all. It could be another virus, but not one that I've ever seen.

The round shape implies they are forming from the inside, like a balloon. That prompted my gas bubble question. However, I think you mentioned, there could also be gas from bacteria. Xenomas are still a possibility. Can you get clearer pictures?

I think the fish is either going go in neoplex or kanaplex today. Not sure which would be better, or if something else would be more appropriate (such as some anti fungal)

That’s weird. I’ve seen swelling on these further up when they’ve been allowed to gulp air, but the only thing back there are intestines.

IDK since I’m not sure what is causing this. Metroplex is supposed to have some affect against anaerobic bacteria and internal bacteria are sometimes anaerobes.

Sorry to hear. Do you have a microscope? Looking at some of this under a scope might help at least rule some things out.
