What can I do to save this blasto?

Title says it all.

Something happened to it when I went on vacation for 3 weeks in July. Hasn’t really recovered or anything has changed. Still has a mouth and I occasionally see feeding tentacles

Target feed it twice a week. Make sure your mag is above 1350, it should come back.
WHich I do.. What about placement? It was in the middle of the tank on the sandbed.

Just moved it to the side with some shade

I keep mine on the sandbed. Kill flow and feed mysis. They are tricky to feed ime.
 
With a frag in that bad of shape I would stick to very small particles foods and only if you see a feeding response. All LPS I've ever had loved reef roids, don't see how it could "kill" a blasto?? I would look into reef nutrition oyster feast, turn your return pump off but leave the powerheads on and add some to the tank. If you get a feeding response then turn off the power heads and gently target feed the blastos. All my LPS loved that and reefroids. Once the coral starts putting on size is when I'd recommend a larger meaty food like mysis.
