What disease could this be?

My friend has a betta fish and sent me this picture. I’m not sure if many on here are super familiar with freshwater but I thought I would ask: what do you think this disease is?

He said the betta just mainly stays in the bottom of the tank except to go up for air

This is bacterial likely from poor water quality and epsom salt at one teaspoon per 10 gallons will help but may require additional treatment using Maracyn oxy

Should he still do his water changes like usual (once every two weeks)?

That could also be a fungal infection from an injury.

Yes and no tap water which causes health issues with betta and other labyrinth fish
