What disease is this? Clownfish

I lost my poor little clown fish today. I have a 20 gallon hospital tank set up and treating some of my fish in it. I am quarantining all my fish for 76 days to keep my MD fallow. Treating with copper at therapeutic levels, because ich!

The clown fish below has been with me since January and has always had blemishes on the side near his gills as well as a white spot pictured below. It wasn’t until I fresh water dipped him before treating with copper the white stuff came out and left 2 holes on the side of his gill. He has been in the quarantine tank for 3 weeks now and has been eating and swimming great until this morning where he didn’t eat, hung out at the bottom and later succumbed.

All fish in the 20 gal quarantine take: another clown, 4 fire goby, 4 cardinals, 1 social wrasse, and 1 lawnmower blenny. I have a Gem tang, a blue throat, and a hippo tang in a separate tank. All the fish look happy and healthy for the most part, I’m sure they prefer being in the larger tank.

Any ideas what this could be?

If you think it’s brook, do you recommend me preforming a formalin bath on all my fish after copper treatment. My copper treatment ends on Sept 25th?

My main goal is to eradicate any and all diseases my fish may have before being put back into main display. So after the copper treatment I was planning on treating for any possible flukes.
