What is happening to my torch?

Context:
3 gallon aio
Only other corals are a small Duncan.
Had in there since may and he was doing great opens up every day and looked amazing. Then I wake up this morning and I see this. One of the head is just gone. Bear with me I am new to the hobby but is it just closed or completely dead. Both nitrates and nitrites are 0 ph is good. Water temp is stable. Flow haven’t changed. What is going on? Should I chop that head of?

This is heavy polyp recession and likely caused by stress from:
Low calcium levels (less than 400)
Too little light and/or water flow
Elevated Phosphates
high temperature
low salinity
change in lighting ( you want medium light and water flow)
Starvation - Feed Mysis shrimp

Everything on your list is in check but I haven’t been feeding. I got some mysis and have feed him a week ago and seemed to open more and respond well. I forgot to feed him for another week and another polyp died. How often should I feed him?

only feed like once every 6-8 weeks or so…
i find benepets to be quite useful for this.

the other two polyps seem to have healthy flesh,
so i’m guessing it’s not bailing out due to parameter swings.
but it’d still be a good idea to zero in on what caused this…
your low numbers aren’t good, if they’re truly zeros..
you want some nitrates and phosphates

Nitrate=0 is not good. I bet your PO4 is zero. What light are you using?

I’m using a hipargero aquarium light 3rd gen. If I need to dose nitrates and phosphates what products do you recommend?
