What is my rock flower anemone doing? Please help!

At first it the anemone started to contract into a point in the middle (first two photos) but then the tentacle looking things split into like groups (third and fourth photo)
I’m not sure it’s a water parameter problem because all of my other anemones/corals are doing just fine but I can provide parameters if needed.
The last photo is what it originally looked like.

Water parameters size and age of tank and lighting will be helpful.

Nitrate: 10-15 ppm (a bit high because I just re-arranged shells)
Nitrite: 0 ppm
Ammonia: 0 ppm
PH: 8.1
Salinity: 1.024
Phosphate: 0.25-0.30 ppm
Calcium: 480 ppm

it looks even deader now but it still reacts when I touch its tentacles

Unfortunately I think its gone.... or close to it.

any idea of what went wrong for future reference?

it could have been a disease of some sort because all of my other coral are doing just fine but if any red flags stand out to you please do tell.

When things like this happen with nems I always suspect a bacterial infection. I keep a healthy supply of cipro on hand and employ @Eagle_Steve method of dosing when needed.

One thing I noticed is that you left out the age of your tank... based on the rust colored algae on your sand my intuition says it isn't very old... nems do typically require a more mature tank that has gone through the ups and downs of not just cycling but a bacterial and micro fauna adjustment. Yes some people are successful without waiting but the vast majority are not...
