What is on my wrasse?

That looks like it could be a bacterial infection. More history on the fish would be helpful.

Hey jay, the fish was added into the tank about 10 days ago and it first started by looking like the scale was peeling off so I just figured he bumped into a rock or scratched it on the sand.

That looks more serious than just an incidental scratch. Remember, reef fish are designed to swim around coral reefs without getting hurt, so when a fish shows up in an aquarium with big skin lesions, something else is likely going on.
