Old Wheat Thin with mold? J/K, the fact that it is perfectly square is kinda funny to me and sorry for being no help.
 
sure looks like a chalice to me. clearly a fragged one but it is a chalice. been wrong before though so take it for what its worth lol
 
I received a frag of this awhile ago. It's a slow grower for me and turned red with white eyes under high light.

Man these "chalices" drive me nuts when it comes to IDing (them and Mussids)... my guess would be an Echinophyllia sp.?

Whatever it is, it's morphed very nice for ya :D

I got this from Will in January (I think). It is a frag from the colony pictured. This picture was in late March when I had just moved from one tank that had just T5's to another with 250 Aquaconnects. It has since gotten larger and the eyes on the top are a light yellow. I'll get another picture of it. It is looking very nice.
