Looks like ich...
Unless the spots get so bad you can't count them, then it could be velvet.
Possibly skin flukes....but those are usually on the body.It doesn't look like Ich to me.. The spots are larger than ich and oblong.

I've been treating with ParaGuard for the last 2 weeks, then this morning I found 4 fish dead. I did a water change this morning after this happened.
Then tonight I found 5 more fish dead, and I found these white oblong spots on my pajama cardinals. These spots were not there this morning.
All of my cardinals seem to be fine, none of them have died yet...
Everything has been fine for the past 2 weeks, up until today.

I still think its ich but with so many deaths so fast it could be velvet. Any fish swim into the power heads or hiding and not eating?
It looks like ich but the sudden deaths are what's throwing me off.They are holding tanks, total water volume of 90 gallons.
I know it's not Velvet, I've dealt with velvet before, these spots are WAYY too large to be velvet.
And no, I've seen no odd behaviors at all.. Just sudden death.

Be flukes, unless advanced stages. Look on all the fishes eyes... does it look like they have a film over them or are they clear?
Sometimes it looks like the film is peeling off the eye. This would be flukes. Are your fish darting across the tank, scratching on the bottom or rocks, sitting in front of power heads, or breathing rapidly? This could indicate gill infection pointing towards flukes as well.
Without a closeup photo/video it’s hard to tell. Looks more like Lymph. to me, if it is not ich as everyone mentioned. I have had some LARGE ich infections in the past. If your fish/tank is new, ich can kill a stressed out fish acutely.
Regardless, given the history and condition of the fish, I would remove and treat all. Good luck!
