What is this on my ruby headed wrasse?"

The only thing on the fish is right behind the lt gill looks to be under the scales. Other spots are in the water or on glass sort. Just got them out of copper QT for last 2 weeks and did not notice this until later that evening when it calmed down some. Have watched for 2 days. No change, swimming fine and eating.

Looks like an injury, possibly an infection. Has it been slowly increasing in size or relatively sudden?
 
No change. Got him, a redline wrasse and a fox face from being qt in copper at LFS. Moved them into the 55gal observation Friday. Late that evening notice this but was thinking maybe sand so have been watching. No change and he let's me get closer so got at pic. The redline has stayed hiding in the sand until this afternoon for about 30minutes. Now in sand again. The most active fish. Keeps going in a cave area with foxface making me nervous it will be stung.
