Sorry for the sideways pics but it still looks the same. It is about 1 1/2"-2" wide and 4"-5" long.
 
Yes it's a Polyclad Flatworm. Can be predatory to clams, snails and some corals.
 
I'll try to capture it tonight. I have lost several acan colonies and a open brain in the past couple weeks. Maybe he's the culprit.
 
I've been reading up on the polyclad flatworm and it seems as it only eats clams and snails. When I noticed it last night it was completely wrapped around my new golden torch.
 
I have only read this as well but I know a hobbyist that found them also on his Euphyllia (can not remember which type) and I have also heard of other hobbyist having issues with them on Hammers, Frog spawn. Considering the large number of different types of flatworms, it does not seem unrealistic to assume there are some types that might become opportunistic feeders in the absence of clams or their perferred snails.

I have slowly lost all my snails (now I know why), so this could very well be. I'm going hunting tonight.
 
I caught it! I finally caught it.
After countless nights of searching the tank I finally caught the nasty thing.
 
Congrats! I have/had some polyclads. I knew they were in this hole so I shot boiling water at them.
