What is this?

I just noticed that my foxface appears to have something wrong. He has a kind of thin mucus like coating, that when he quickly swims off, leaves sheets of this mucus-like stuff floating in the water, like little milky clouds. Is this velvet or what? Should I remove him from the tank.

I have had this foxface only about 4 to six months and the whole time he hasn't looked quite right. he hides and changes color to a blotchey grey/white with dark streaks. Seems to be stressed all the time, and yet I don't see that he is being bullied?

Any ideas of what is going on?
 
Did you QT him when you got him? Any other symptoms? Scratching, flashing, heavy breathing? It could be that he's producing so much slime coat that it's being swept off when he moves quickly. I would think that if he's doing that, it's to fight something off. @Humblefish will be needed for this one.
