What is white spots on puffer

Unfortunately my friend I cannot see anything wrong here. Try multiple pictures from different angles. It would also help if you steer our eyes to where on the fish the spot is.
 
I can see a little of what might be a little lympho or a nip on the tail, but I agree that fish needs some serious feeding and soon. Puffers need big meaty foods (clam, mussels, krill, etc), I'd start out with smaller pieces several times a day and work your way up. Also, soaking foods in a vitamin supplement like Zoe or Vita-chem is also a very good idea.

Have you seen any white-stringy poop? If it's eating well, but still very thin it could be suffering intestinal parasites. White stringy poop would be the indicator.
