What is wrong with my hammer/frog spawn

I don’t know what’s happening. The head went all stringy for a few days then this happens? Is it dying?

I don’t know exactly, I just get my water tested at the LFS I go to and he just tells me if they are ok and stable which pastime I checked were. The only thing I do know is that my phosphate is at 10 ppm and that I dose ~4 ml of 3% hydrogen peroxide to help with clarity and disease. I did recently change to dosing 1/3 of a spoon of polyp lab reef safe medic and added a 6 line wrasse but this is all after this started happening
 
I'd highly recommend performing your own testing. Without this knowledge, you will have trouble diagnosing most issues you come across in this hobby.

I know I’ll buy some soon, I’ve been strapped for time and money this week so I’ll try and get it soon
 
High po4 is no bueno. And if you are lowering it to fast it can stress and kill corals. I would recomend buying your own test kit. And make sure you are not lowering it slowly.

10 ppm phosphate is not too high and it’s been at that level for a while as well as the fact that all my other corals are doing fine
 
I'm pretty sure you mean 0.1 ppm phosphate. That other hammer does not look terribly happy either. Can we see a full tank shot?
 
Not sure about the h202 dosing, maybe someone else can chime in.
It’s used for tank clarity and to inhibit/get rid of common dish disease like ich, Brooke, and velvet. It’s dosed in the ratio of 1ml of 3% per 5 gallons. You can’t use it while running UV or Ozone. That’s pretty much all of it. Here’s an article.

- High alk and/or PH

What is age of your tank? From looks of rocks- new
What is tank temp and salinity?
What is your ammonia and nitrate level?
Are you using tap from sink or RO water?
How did you acclimate the coral to tank?
Any fish in tank ?
What test kit(s) are you using ?
 
This coral is salvagable if you can assure parameters are corrected.

It’s about 2 months old, the temp is currently 78, the ammonia is 0, the nitrates are 0, when I do water changes I get water from LFS, otherwise I use distilled water in my auto top off, I temperature acclimated then added a bit of water every 5 minutes for 30 minutes, I have 1 clown, 1 court jester goby, one 6 line wrasse, 1 Halloween crab, 1 blue crab, and a nissarius snail. I test my water at the LFS but he uses a Hanna checker for phosphates and it was at 10 last time and since then I did a 10% water change
 
I assume you mean phosphate is 0.10 and not 10, correct? That doesn’t seem bad as long as it’s stable at that level. Tank is relatively new it sounds like.

The thing is I don’t know if the LFS used a ppm measurer or a ppb measurer so it could be either one. Either way it’s in the acceptable range and it’s been consistent

Now you know ammonia and nitrate levels too or are you just assuming because you were told by your LFS that all is good?
 
How much lighting and flow are on it?
Most of my flow is at the very very top of the tank and not hitting any of my coral or rock directly and I have a single ai prime centered and a little lower down then the max height. (Lighting schedule attached)
