What is wrong with whitetail?

My whitetail tang has started acting very strange. He is jerking backwards and forwards and is really skittish. We have moved all fish into QT Monday and treating for flukes following death of regal tang and fluke signs in the DT. Is this fluke behaviour or could it be something else?
We did FW dip to worst affected fish which unfortunately our yellow tang didn’t recover from and died from shock despite our best efforts to revive him. Devastated at losing these two fish as they were such great characters but worried now the lipstick, whitetail and linespot trigger may go downhill too. No white spots on any fish tho the whitetail did look like he had lymphocytes however this has now almost gone. Anyone had this behaviour before?

He was acting that way in the DT before we moved him too
 
No changes to the tank for a few months. Had flukes months ago but they shook it off so we didn’t fallow the tank at that time. Think we will cupramine after the fluke treatment just to be safe and leave the DT fallow for at least 6 weeks. We had ich before and the symptoms are different this time. The tank was fallowed for months then so we can’t think how it could be that again.
 
It appears to me that this fish is simply pecking on the algae within the tube. To assure he is perhaps hungry, try a piece of nori seaweed on the opposite end of tube. If he goes for it, then you know. There is no condition for rocking back and forth other than in front of a powerhead which generally suggests velvet or inadequate swimming room from a small tank.

The tank was fallowed for3 months after ich. Longer than we needed just to be safe. He’s not trying to get algae, just going backwards and forwards mainly at early and late light changes. We did get a nitrate spike before we moved them but had this back under control. There’s definitely something on his fins but looks more like lymphocytes than anything else.
 
Well bad news. He went last night. The treatment in qt killed the bacteria and we got ammonia spike which has wiped out 50% of our fish. All tangs gone and the rest are not looking great. Fingers crossed they pull through. Now back in DT as the ammonia was sure to kill them. Absolutely gutted

Ammonia spike in a qt used to be common as they were essentially setup with no rocks , sand or anything .
Setup with old tank water , a simple filter and something such as pvc for hiding space .
Dose meds and water changes daily to reduce ammonia .
 
We did all that. Filter sponge from main tank top. Only copperband and linespot trigger left with us
