Not having problems, just curious.

I keep finding these things in my tank. Little slightly-green worms of waste or something? Too heavy to suck up with gravel vacuum, not moving they disintegrate on contact.

Is this just algae clumped together? How does it clump together like that?

Yes I have a yellow tang, sizable

What would be the best way to clean this up? If I gravel vacuum it sinks back to the bottom, even when I suck up sand into my bucket.

If I try and grab it with a pair of tweezers it disintegrates.

Use a long handle cat litter scoop and just scoop up gently and will not fall apart.
 
I would let it be. If you're worried about a little fish poo you may be running too clean of a tank and that may lead to cyno or dinos.

If you're worried about the aesthetic Increase the flow, buy some clean up crew. Hermit crabs do well with this.
