Is breathing normal or labored?
Is it still eating?

I see fin and mouth damage. These are almost always caused by mechanical trauma. That can be shipping, catching the fish in a net, or tankmate aggression. This species of tang is prone to hitting their mouth on things in the tank if scared. However, the tail damage doesn't happen that way. In severe cases, secondary bacterial infections can being to make the damage worse, I think that is happening here.

Agree with jay on mouth rot and/or damage. These guys peck at glass and rocks all day and this one has become raw. Placing in a separate quarantine/treatment tank with no rocks will allow some healing as well as feeding soft foods such as brine shrimp, mysis shrimp, plankton and nori will not inflict further bruising damage and treat with Seachem kanaplex or Neoplex
