WHAT TO DO ?

QT already has Cupramine and Prazipro running for a week now. One of the Koi tang has pinched belly and white stringy poop and not eating .My questions are, #1 can i add metronidazole to the QT with Cupramine and Prazipro in it? #2 Do a big water change ,start with metronidazole til the pinched problem is gone and start all over with Cupramine and Prazipro ?
 
I've never dosed all three at once. Praziquantel is pretty transient, it doesn't stay in the water very long, it gets decomposed by bacteria. If its been a couple of days since the prazi, you should be good to dose copper and metronidazole (but what dose do you plan on using?)

Can you post a picture or video of the tang?

I don’t think there is a good answer to this. There is a fantastic write up of Copper by Humblefish and what it does (ich/velvet) and what it doesn’t. Nonetheless your copper therapy for those is insufficient. Short of giving up on copper, you could only go to tank transfer method.
Prazi stays for 5 days in the water. Pending your timing, it may be gone.
If I remember correctly someone combined copper with API general cure (Prazi/metronidazole).
Of course I am concerned about O2 depletion and immunosuppression. So I don’t know if more medication here is of advantage or disadvantage. Pictures may help.
 
It hard to get a clear picture after dosing Prazipro. Koi Tang with pinched belly and a bump on one side of the belly. Not eating and having white stringy poops. Dosing Cupramine and 2nd dose of Prazipro at this moment. Did a Fresh water dip for 10 mins and found no flukes. Second day of 2nd dose of Prazipro. Should i add metronidazole with Cupramine already in the system once the Prazipro is decomposed ? Or having a separate QT with Focus and metroplex for that one tang?

The stringy feces is likely just mucus from not eating. The bump on one side could be an issue - anything asymmetrical on a fish bears further examination. What dose of metronidazole do you plan on using? I've used doses ranging from 25 to 100 mg/gallon. Usually dosed every other day for three times. You can dose that with Cupramine, but be aware that both treatments cause fish some stress and that will be combined when used together.

The Koi tang didn't make it. Did a 50% water change after scooped the dead fish out without adding any medication back in QT . Wait and see how the rest of the fishes react. Will continue with medications later.
