What's going on with you hammer?

With in the last week I notice that my hammer will shrink up and some brown slimy think comes out. Did water changes but no change for my hammer. But at the same time my frogspawn is growing crazy. All my parameters are stable I did notice that my phosphate is a bit high, it's at 0.42. Could this be the problem?

Has there been any light changes?
.4ppm phosphate is high IMM, in a new tank maybe 0.03-.1ppm, older stocked established tanks, maybe 0.1-0.25ppm...

No changes in light and the tank will be a year old in this July.
