Whats on my Micromussa???

I found what looks like a worm with multiple "polyps" that look like aptasia on the base of my micromussa. The "worm" is not directly on the coral just on the frag plug but it does seem to be negatively affecting it. The white tube on the picture is connected to the aptasia looking thing. Does anyone know what this is and what I should do to get rid of it?
