I've had lionfish look like this after a heavy feeding, but I have no idea what's going on with this little guy.
he isn't new to the system. swimming ok. eating.

Try feeding decapsulated peas to help with constipation. He might also have internal parasites.

parasites would suck. I can catch most the fish in my tank, but not this one.

Could just be it's gorged itself on food which isn't that uncommon in this species. Brine shrimp can help shift a blockage so I'd try feeding them for a few days to see if that works.

Try epson salt 1 teaspoon for every 5 gallons I believe is the formula or a pea

Peas are more an option for goldfish or some tropical freshwater fish species. By all means give it a go but I'd be surprised if a sixline did more than watch it sink to the bottom.

Epsom salt is just a magnesium suppliment and with a reef you should have plenty of magnesium in the water. So once again I don't think this will solve the issue but it can help with freshwater fish.

I doubt it’s internal parasites. If anything internal parasites using make the fish thinner because they’re eating the nutrients. My bet is some form of constipation or maybe pregnancy.

There was a thread recently with the same problem although the op never updated us but I mentioned that sixlines don't get pregnant. It might seem like semantics but pregnancy is where a offspring develops in the body and with wrasse you may have eggs but it's unlikely unless there is a male present. With these fish they often have developed eggs but they are only visible just before they are ready to spawn when they absorb water and swell up, giving the fat look, this is stimulated by the presence of a partner and without one it's not very common.
These fish are supreme micropredatos and often don't stop eating until they pop so it's likely it's just found an abundance of something tasty but we can't rule out a blockage (possibly egg bound but less likely) or disease of some sort, without further info.

Any other wrasse in the tank. I do not think it is parisites same thing happened with mine she was pregnant this can happen with any fish my mandirans and my tail spot blenny look just like this. I have been in the hobby for 20 years but only seen this happen 10 times.
